Surgical Error Investigation
Surgical error at Tseung Kwan O Hospital tied to death of an 85-year-old woman,calls now rising for surgeon's dismissal. Investigation found surgeon showed “confirmation bias” during February 7 procedure for obstructive sigmoid colon cancer.
Operation was supposed to be a transverse colostomy — creating stoma in abdomen to ease intestinal blockage. Patient's vital signs stable post-surgery, but symptoms were troubling. Unusually high stromal output. By March 1,condition worsened with hypotension,tachycardia. Transferred from Haven of Hope back to Tseung Kwan O next day.
CAT scan later showed critical mistake: stoma placed in stomach,not colon. Health declined further. Family faced hard choice, agreed to do-not-attempt-resuscitation. Patient died March 3,three weeks after surgery.
